Academic Probation Acknowledgment Form: Faculty of Arts
If you are on academic probation you must make an appointment with your academic advisor before starting courses in fall term to ensure that you fully understand your situation and that you are taking the necessary steps to improve your grades. It is recommended that appointments take place in July or August once course registation is complete. Appointments can be booked online and may either be for in-person meetings or phone appointments.
Questions or concerns with this form or procedure should be emailed to email@example.com.
Note: OSAP probation and financial holds are different from academic probation status. Contact student awards if you have any questions about OSAP probation status and contact Service Laurier for financial holds.
