Preferred Name Change Form

* Please note, as of October 06, 2020, Preferred Name Form requests will be held until the beginning of November due to government reporting. After November 2, updates will be made to all forms received within this time.

This form is for students who wish to change their preferred name in LORIS.

Your requested preferred name will appear in LORIS as well as all MyLearningSpace (MyLS) classroom lists. Note, you will need to receive a new one card with your preferred name for exams. If you would like your preferred name displayed on your MyLaurier email and in Zoom, please contact the student help desk.

Please be advised that your legal name will remain on all official documents (transcripts, verification of enrolment, tax forms, degree parchment, etc.).

During every term a reconciliation with the Ministry of Education is performed and your name will be changed back to your legal name temporarily until the reporting is complete. Please contact avelanoff@wlu.ca if your preferred name has been changed and you have further questions.
