Complete this form if one of the following occurs:

• You have two final exams scheduled at the same time.
• You have three final exams scheduled within 24 hours (e.g. if you have an exam on Tuesday at 7 p.m., on Wednesday at 8:30 a.m., and on Wednesday at 3:30 p.m.; this would be within 24 hours).

This form is not applicable for midterm examinations and must be filled out three business days before the first problematic examination. Any submissions after this date will not be approved.
