This form is for students in the Faculty of Liberal Arts and Faculty of Human and Social Sciences who wish to change their legal name in LORIS.

If you are requesting to update your name for internal use only, (ie. preferred first name, nickname, remove middle name, etc.) please do not complete this form. Please complete a Preferred Name Change Form.

The name as it appears in LORIS will be reflected on your academic records (Transcripts, Verification of Enrolment, etc.).
