College of Social Workers Confirmation Request Form

If you have completed all required courses for your MSW degree, marks for all courses have been applied in LORIS, and you have applied to graduate (but you have not yet convocated), you can request a Confirmation of Completion letter be emailed to a provincial College of Social Workers.

Please note that the Ontario College of Social Workers and Social Service Workers (OCSWSSW) is no longer processing registrations prior to convocation. More information can be found here.

After graduation it is your responsibility to forward a copy of your official transcript to the appropriate provincial college. The Faculty of Social Work does not facilitate this process.
